Benchmarking to Measure Your Practice’s Success

I’ve written extensively on really taking command of the direction of your practice, and your life as a powerful economic stimulus.

In reality, it’s all you have in private practice. There are some really simple ways to do this such as benchmarking, but before we talk about that, here’s an “Economic Summit” reality check-up.

If I told you right now there are doctors like you doing 7 figures cash/year, would you believe me?

If not, why?

Likely, it’s a very limited worldview on what you can really accomplish with all the correct systems and tools. And far too often, this is so defeating if it takes place early in ones career.

If I also told you Concierge MDs are collecting 10-15k per patient per year cash, and these same patients gladly pay, and in fact request that type of healthcare, and also pay health insurance premiums, what does that say about how you value, and price your services?

We’ll touch more on this very topic, with some really incredible guest instructors for members, in the near future. Some absolutely brilliant ways to look at life and practice in this brave new world. Yes, even in a down economy.

Well, here is another huge economic management tip.

Regular benchmarking of your results against your goals helps put you at the head of the class.

There is a real easy way to get this done. This tip alone might put another 10K in the income column this month.

The first thing to do is take your list of basic daily, weekly and monthly targets. Higher performing CEOs/CFOs do this daily. Use your white legal pad, but better yet for this one is an Excel spreadsheet (because you can graph it easily and tweak it in all sorts of ways).

My basic daily targets are body weight, miles run or cycled, hours skied, services and income, dollars saved. Yours might be totally different, as it should be. You really can do some incredible things with these numbers, but resist that temptation in the beginning.

Put your goals or targets (BOLD) on one axis, and simply enter your results on another. Save room for notes, photos, and maybe even video clips.

Next, post this somewhere you can see it all the time during the day. Admittedly in the office, maybe it’s in your portfolio, which you pull out during the day, or simply, hidden on your desktop or handheld. Just be sure to look at it at least once daily.

Here’s why this is so powerful. Because it’s a glimpse of right now.

Many people who have trouble setting goals find new life with this method. It keeps you in the moment as well. For us type A’s, it can actually be a better way of dealing with “Futurizing”, which some might say is an unhealthy psychological trait.

But to make sure you’re not deluding yourself, if you use daily targets, they must be the correct fraction of the weekly/monthly/yearly whole.

Just by doing this daily exercise, some great things start to happen. Many times, I have found breaking so many things in life down into daily pieces really simplifies accomplishment and helps move me forward instead of stalling or going into overwhelm.

Now, you are forcing yourself to look at your goals. In fact, now you have actually set goals!

Next, you are inspecting what you expect, automatically!

And finally, you are really taking advantage of a tremendously powerful principle.

That which can be measured, like your results, can be improved!

And herein lies the most powerful tip of all.

Continuing to improve your results and changing your targets to suit your desires in any endeavor is the ultimate in human accomplishment.

This single hallmark of a powerful life is shaping and molding that which we can control. It is creative energy harnessed. It is fulfillment vs. frustration. Happiness vs. resentment and regret.

When combined with all the other key components of Living and Practicing by Design™, it adds a totally new dimension to who and what you can become.

The key lies in customer service… in which your staff is your number one customer.

Ask for employee input, and actually take it into consideration. Your staff has an intimate knowledge of what makes their work environment positive—and what makes their professional lives difficult. Find multiple ways to invite input, both public and private. When staff raise a good idea, implement it!

Get to know the personal lives and preferences of individual staff members. Learn their birthdays, the names of their spouses and children and dogs, and their favorite sports teams. Spell their names correctly in memos—you’d be surprised how many private practice owners don’t bother with this detail!

Don’t assume that staff are happy in their current positions or even career fields. If you are having conversations with staff (formal and informal) about their career goals and hopes for the future, you’ll be able to support them in relevant milestones. Just as important, you won’t be caught off guard by a sudden career change.

Write Down This Key Advice for All Private Practice Owners—Instead of Ignoring the Business Areas Where You’re Weakest, Outsource Those Tasks So They Become Strengths. Here’s How to Do It Effectively.

Our culture tends to foster a go-it-alone mentality. You know, just pull yourself up by your bootstraps and be willing to learn anything and go the extra mile—that’s how you gain success in any business. Private practice owners tend to take this to heart!

But the truth is that by trying to handle it all yourself, you’re spreading your resources too thin. In the end, successful private practice owners make their practices succeed by building an incredible team. And that’s more than just the staff in your office that you see every day. Your team can include freelance professionals who handle mundane or specialized tasks for your practice so that you can focus on your strengths.

By outsourcing your greatest weakness, you’re raising the bar. Instead of expecting that those tasks will be handled “well enough” (i.e., in an average way) through your own efforts, you can build a relationship with trusted professionals who will far outperform you—and thus, let you focus on YOUR work with patients, where you really shine.

The outsourcing process, in a nutshell, is actually very simple.

First, identify which tasks are currently a liability to your practice because you can’t or won’t do them effectively. For many private practice owners, these could include payroll, marketing, or website management.

Next, research two to three options for getting these tasks done by someone outside your office. This is probably the easiest step. Even if there aren’t great resources in your local area, consider that many tasks can be done remotely, thanks to the Internet. Private practice owners can form a relationship with remote freelancers by talking with them through Skype or teleconference software.

Meanwhile, you’ll notice during this research phase that you will begin to have all sorts of mental objections to outsourcing. Make a note of these thoughts. Some of them may have validity (see the next paragraph) while others are based on emotion, not a logical justification. Talk with someone you trust who can help you think it through.

Next, before you make a hiring decision, do what you can to minimize risk—and there’s a lot you can do. Ask other private practice owners and related professionals for recommendations, instead of trying to reinvent the wheel.

When you find a freelancer you like, ask for the contact information of satisfied clients (and call at least one to confirm the testimonial!) and ask to view samples of the freelancer’s previous work. When you’re ready to hire, be sure you start with a written contract for one small, quick job. That will allow you to evaluate the freelancer’s work for yourself before making a substantial financial commitment.

It’s possible to be weak in a significant area of business and still succeed, as long as you’re able to put together a strong team.

Diabetic Neuropathy Patients

Treating Patients With Diabetic Neuropathy

If you treat Diabetic Neuropathy Patients chances are you are no stranger to making diagnoses of neuropathy. While some patients (even those who do have nerve damage) might experience no symptoms at all, about 60 to 70 percent of diabetics experience pain, soreness, loss of sensation, tingling in the extremities, and even digestive problems—or other conditions related to organ complications—all symptoms of peripheral neuropathy. Diabetic Neuropathy is one of the most common causes of neuropathy overall.

A patient’s risk of developing diabetes-related neuropathy actually increases with age and extenuating health considerations (such as being overweight), partially because patients who have problems with glucose control for extended periods of time—25 years or more—are more susceptible.

So, what causes Diabetic Neuropathy symptoms? Research is occasionally unclear on the subject, but it is generally agreed that exposure to high blood glucose (high blood sugar) has a negative effect on nerve condition. Of course, this is in addition to other conditions or lifestyle factors commonly associated with causing or exacerbating diabetic neuropathy, such as injury, metabolic inconsistencies, inherited traits, or substance abuse.

There are a few kinds of neuropathy associated with diabetes, the most common being peripheral neuropathy (this is the type usually referred to when people simply say “neuropathy” but we’ll get to the other types in a moment). Peripheral neuropathy is characterized by pain, numbness, tingling, and loss of motor function, among other sensation-related symptoms. This type is written about extensively, and can greatly impact quality of life for its sufferers. Most treatments available to medical practitioners target peripheral neuropathy the good news is, there are plenty of ways for you to treat this type.

Focal and proximal Diabetic Neuropathy result in muscle weakness and pain, and typically target a specific nerve grouping. These types of neuropathy are commonly characterized by weakness in the legs, causing difficulty standing and walking. This type of neuropathy often accompanies peripheral neuropathy, so be on the lookout for patients who experience weakness alongside loss of sensation or soreness.

Autonomic neuropathy, as the name implies, causes changes in autonomic bodily functions. These include bowel and bladder functions, sexual responses, and digestion. Autonomic neuropathy can be life-threatening in extreme cases, as it also affects nerves that serve the heart, lungs, and eyes. Especially troubling to diabetic patients is the resulting condition of hypoglycemia unawareness, which can reduce the symptoms most diabetics associate with low glucose.

It is recommended for any diabetic patients to receive at least an annual foot exam. If neuropathy has already been diagnosed, a patient’s feet should be examined much more frequently. Additional to diabetic amputation concerns, you should test your patient’s protective sensation by pricking their foot with a pin, or running monofilament across their skin. If your patient has lost protective sensation, he or she could be at risk to develop sores that might not heal properly, leading to infection. For other types of neuropathy, properly-trained clinicians should perform a check of heart rate variability to detect how a patient’s heart rate changes in response to changes in blood pressure and posture. Ultrasound imaging is also useful to diagnose autonomic neuropathies and to ensure other internal organs such as the kidneys and bladder are functioning properly.

To control diabetic neuropathy, it is important to advise patients to maintain a tight blood sugar control and a healthy diet (this methodology is advisable for diabetics in general, of course). Even if a patient does not have symptoms of neuropathy, regular checkups are wise. You may think your private practice is pretty good at customer service for patients. But you can take it to a whole new level when your staff learns to serve each other.

Do you know a lot about customer service? It’s definitely the bottom line when it comes to marketing a private practice. But what you may not realize is that customer service isn’t just for patients and new prospects.

Imagine a world in which your staff treats EACH OTHER like valued customers. A world where you’re able to retain staff and keep morale at a steady high.

You can create that world! And your patients will sense a positive shift in your practice culture—which will keep them coming back.

A private practice with a customer service culture has several elements working for it, which you can easily begin to emulate.

First of all, it’s the simplest things that change company culture. “Please” and “thank you” go a long way toward building morale and helping every staff member feel valued. You can suggest at your next staff meeting that everyone remember to use these simple words with each other, not just with clients. But more importantly, you can model this new behavior for your staff. They’ll do what you do, not what you say.

Second, practice listening with respect, not impatience. Too often we are so busy and wrapped in details that we barely acknowledge staff with a nod. What if you made it a practice to always make eye contact, listen fully, and value that person’s contribution to your workday in every exchange? You don’t have to do it perfectly, but there’s a good chance that every respectful listening exchange will have a ripple effect among your staff.

Finally, find ways to offer timely feedback, both positive and constructive. Begin building feedback opportunities into the workweek rather than saving them for performance reviews. Most important, be aware that feedback shouldn’t come just from you as the private practice owner, but from staff member to staff member so that they are supporting each other. Teach them to “catch someone doing something right” and provide concrete ways to share those moments.

Think of how it feels to walk into a dentist’s office, for example. There are some pretty standard things that you expect because they’re the same in almost every dental office: popular magazines, photos of smiling people on the walls, an easy listening station piped into the waiting room.

But imagine that you walk into a dental office and experience pleasant surprises. What if the dentist stepped out into the waiting room personally to introduce himself and say how nice it was to meet you? Who would you rather have serving you? The tired customer service rep who is merely going through the motions? Or someone who reaches out to you sincerely and asks how to best serve you today?

Patients can tell when your marketing is heartfelt, and when it’s just going through the motions. In private practice marketing, choose to make real connections every time and you’ll enjoy better referrals.

The “Whys” of Private Practice

You could argue that without purpose life is extremely difficult and the same of course is true of your #privatepractice

Purpose. Without it we lack reason to do what we do every day.

Unless you know precisely what your purpose is you will find private practice difficult, and at the very least unrewarding.

Make no mistake about it there are multiple reasons that we have private practices. One, it is a business and the purpose of your business is to generate income. There’s nothing altruistic about this, this purpose is just reality! The purpose of generating significant income is to provide value to those you love and all the experiences you will be able to bring to your family.

But more than this, having a higher purpose allows your community and thus your patients to identify more completely with who you are and why you do what you do.

We first have to be clear with ourselves. Why do we get out of bed every day and do what we do for our patients in our communities? The real reason as most healthcare professionals went into their chosen healing art, to enhance the lives of those we serve.

And there you have it! This is the highest purpose we could ever aspire to.

So if this is the case then why do so many find private practice or indeed any business so difficult?

The reality is most people don’t take enough significant quiet time focusing on and improving everything they can to enhance their purpose!

In fact, as the great business philosopher Jim Rohn once said, “Lacking enough “whys” or reasons to achieve our goals is a certain pathway to failure!”

Personally I agree!

This viewpoint has always served me well!

Marketing is key to rapid and sustained practice building.

As consultants, one of questions that we get asked all the time from physical therapists, chiropractic physicians, as well as medical physicians is: what is the quickest way to build professional practice or rapid practice building?

Is this even possible?

The answer fortunately is yes! Practice building is a lot like building any successful business. It takes design, very detailed and well-developed plans, and much work involving an incredible assortment of details.

And unless you have lived it, you really have no idea the magnitude and depth that effective practice building can take.

But more than that it takes an extraordinary hand (at the helm and I certainly hope that is you!) at balancing all the above with an effective and powerful marketing plan. Like it or not the marketing plan is where too many fail in any business!

Obviously then marketing is so key to rapid and sustained practice building. Many critical parts need to be taken care of. For example, what types of stay in touch programs do you have in place for your private practice? When was the last time you worked on really effective mechanisms to survey patients on what they want and desire from their private practice owner?

Also, when was the last time you sat and listened to your staff actually answering the telephone? Is it like you have scripted for them? Or do not even have telephone scripts or effective staff training?

By now you see where the conversation is headed. Rapid practice building is entirely possible, if you’re willing to do all the above and much more.

So much of practice building in this day and age, is centered around the specific needs of customers.

And you know, firsthand how difficult this can be. Much like running any business to be extraordinarily challenging, I gets more complicated all the time.  Yes running a powerful private practice requires multiple skill sets. It also requires extensive management and an extraordinary team effort!

But more than this requires your personal passion. Your passion for helping others with the least invasive care possible. Your passion for sticking up for your patients rights. Your passion for delivering care with the least amount of potential harm first. Your passion for educating your patience and your community.  See where were going here? Begin “Partnering” With Your Patients Today

As students and even daily in our practices as clinicians we spend lots of time, energy and resources designing treatment plans for our patients. Now of course so much of this is just essential to good practices and indeed excellent patient treatment planning should be our primary line of thought, just after completing our initial consults and exams.

But far too often this process can become cumbersome.

And now more and more we train our students to always go “by the book”. Do what the book says or politicians more and more seek to mandate. For too many there is never true partnering with their patients!

We understand firsthand that just the documentation time and record keeping alone can extract a huge toll on us as private practice owners.

You know, what’s the “standard of care” for Mrs. Smith and her ailments? What if any, paid benefits do you receive for providing your care and what’s not covered by third party pay? What requirements must you fulfill to document all of this for patients?

What I find is too often what can and indeed does happen is empiricism and common sense take a backseat to administrative requirements and the associated BS.

I’ve been at this long enough to remember the primary reason we did what we did in private practice especially was to simply partner with our patients, find out what their goals are then simply act accordingly.

Now there is always someone to second-guess your treatment plans.

But there is a better way! And that is to start partnering with your patients, today!

What does partnering look like? Well initially by matching patient goals for care as close as is reasonably possible.

More than this though partnering with patients means putting the responsibility for healthy behaviors squarely on them. Partnering also means clearly outlining, then agreeing on mutual obligations.

No doubt this is a huge part of what you do already but try taking all of this to the next level.

Really starting today, spend more time realistically engaging with your patients on not only some acute care when needed but also chronic care direction and stability.

Make sure patients know exactly what your skill sets are and exactly what you can do for them.

Right this minute, patients are searching for you. They are looking for a private practice owner with unique skill sets that you possess.

You see these patients are very frustrated with the care they are currently receiving elsewhere.

However, unless you tell your existing and prospective patients exactly what your skill sets are and exactly what you can do for them, practice will be a lot harder than it needs to be.

Of course there are multiple ways that you need to approach this. As an astute marketer you already know the necessity of having multiple forms of communication. That’s why your marketing has multiple outreaches. You’re touching your community both on and off-line. You have a presence, you are the trusted authority.

But did you also know the more you can emphasize your precise skill sets in practice with simple tools, the more effective and profitable your private practice will be?

There is no better time to do this then on the first visit.

And rather than this taking the form of some heavy-duty hard-sell marketing push, it should simply take shape by your excellence as a clinician.

Make no mistake about it.

Every patient knows how well you train your staff and they know if they are caring as well as knowledgeable.

The same patients also know how well you’ve treated yourself and your approach to them as a highly skilled professional.

Keep yourself engaged in practice using your existing skill sets then continuously adding to them over time will increase your career opportunities dramatically.

This does not have to be complicated. Start by simply reviewing your consultation and most especially examination skill sets and be MORE engaging during your patient’s first visit!

Clinically, nowhere is organization and cleanliness more important than the specialty practice. The reason for this is that, in specialty practice, we’re often times treating patients who are or have been seriously ill, suffering from infections, diabetes, and the like.

For something so simple, why does this practice building basic concept escape so many private practice owners?

After having visited many private practices throughout my consulting career, I can tell you that the single biggest destroyer to organization is paper. After all, there are files, journals, faxes, and printed reports, etc…

The good news is, with some very simple practice building basic steps, you can make extraordinary progress.

Simply by organizing files–but most especially not saving and eliminating any paper–you can make a huge difference.

You’ll find it makes a great difference to have specific filing places for things such as EOP’s, submission reports, day sheets, and other business records which only need to be kept for relatively short periods of time. Better yet, get as many payers on paperless as you possibly can.

In this day and age, a high capacity scanner and excellent software is one of the keys to organization.

In this media-driven world, one of the biggest mistakes today’s private practice owners make is neglecting some of the practice building basics that are available, virtually free, and for the asking.

Let’s face it, all successful businesses are built upon relationships. Relationships are made by meeting more people. One of the easiest ways to meet more people is to take advantage of local media, social events, and experiences.

Right now, in your very community, I bet there is a newspaper editor who would welcome your press release with your photograph.

Likewise, there is a support group leader looking for a speaker.

There is a local radio show host looking for a guest, or a local community TV station looking for a health expert.

There’s also the local business owner who needs expertise in helping his employees maintain their health and productivity.

The cost to do these things? They are all virtually free. So why are they so often neglected?

The reason for this is that they are never tasked and never assigned. This is also why so many private practice owners will fail, or never do as well as they could.

So what’s the solution here? The solution is to vow to make a list right now of all the things you know you should be doing—but are not.

And, honestly, the ideas I have just given you are easily worth six figures a year or more. It’s critical that you, as a private practice owner, begin to think like this on a regular basis.

One of the things that we talk about all the time with regard to growing and making sure your private practice has longevity and continued profitability, is staffing.

This is so crucial and so overlooked in all businesses except the most successful. Your private practice is no different.

One of the things you can do to ensure good staffing is to make sure you have a private practice bullpen.

Without any doubt you must have team players that can rotate through the various positions, just like a major-league baseball team.

Simply having basic systems in place along with a fantastic manual and understanding of key positions for your players’ goes along way towards moving your team ahead. Just like in Baseball it can make the difference between first or last place.

One other key point is using the bullpen concept allowing you to rotate your players. Just for fun, periodically try rotating your staff through various positions within the practice.

This prevents job fatigue, keeping your staff fresh and engaged.

Amazingly, it also naturally helps produce practice growth. You’ll find patients responding in kind as well.

Mixing up the routine is great however, just as in major-league baseball good managers must be more specific. You should know the strengths and the weaknesses of all your players.

There may be some players’ that are unable to be the pitcher, as that of course is a highly specialized and skilled position. There are also some that no doubt will excel as your designated hitters.

Even if your practice is small you should have access to people you can call upon in the event of vacations and illnesses etc.

The nice thing about this is it only requires some planning. Make sure you continually set the stage for practice growth.

Unless you have the capacity to pull this off, you’ll find private practice to be way more stressful and far less rewarding
