The Magnetic Power of Common Courtesy

Recently, I had the opportunity to travel to St.Louis. This really turned out to be a fantastic trip! I got to mingle with other docs and to spend additional time developing plans for the future of Medicine. Now, I spent four years in the Midwest, but admittedly it’s been some time. I had forgotten how common courtesy seems to permeate the entire culture of the Midwest, in stark contrast to those of the Northeast.

Again, I don’t think this is just perception, because last week when I related my experiences to various colleagues they reported similar encounters. One even called us Yankees the “Grunt Society”.

Upon arrival at my hotel, I asked the concierge if Busch stadium was within walking distance. Fortunately, not only was the stadium within walking distance, but so were the St. Louis Cardinals Hall of Fame, the famous Arch and National Park, and riverboat dock. Fortunately, it was a warm sunny afternoon so I ventured out for about two hours.

My first stop was the stadium. I spent considerable time admiring the architecture and how well it is cared for. As baseball stadiums go, this one is magnificent. While wandering through the stadium store, and wondered if my friends would consider me a traitor for bringing home an Albert Pujols T-shirt.

Next, I ventured down towards the river. Along the way, I had the opportunity to stop for a cup of coffee. Now, this was similar to any ordinary coffee shop in any city however the counter clerk could not do enough to make sure I was completely happy and satisfied. I was actually blown away by his full attention to sure that everything was just right.

I spent some time walking through the National Park along the river. Having not been to St. Louis in 17 years, I had forgotten what a nice place this is!

On the way back to the hotel, I stopped at a small city supermarket, and had a very similar counter experience. I’m beginning to think, “…is it just me? Am I more relaxed than usual, or is this place really special?”

The next day, hotel restaurants were naturally jammed during the breaks so I ventured outside and down the street once again. I stumbled upon a local restaurant in the business district, with the line almost all way out the door. I decided this would be a great place for lunch. Not only was the lunch fabulous, but also the service was absolutely impeccable. Everybody from the front counter down to wait staff wanted to make sure everything was just right, and I was completely comfortable.

In retrospect, it seems like the entire five days was magical. Not only did I get to spend some time with very special people from 20 different countries, but also was made feel right at home everywhere I went.

There is of course, a message here. People remember how they are treated at a more emotional level than they do the actual product or service they are seeking.

Therefore, it is no secret that this type of attitude, a feeling from the top down in your office has substantial impact on patients to return, refer their family and friends. and take care their finances.

This of course is a key element of Living and Practicing by Design. As much as possible, procedures must remain patient friendly. They must be easy for the staff to execute. They must be clear, and easy to understand. Patient benefits must be stressed, but above all they need to be conveyed and delivered with exceptional courtesy.

So, I would recommend that you share this message with your staff as I have. This would be a good opportunity to review point of contact systems and procedures, especially telephone and front desk procedures. Spend some time here, watching for tone of voice, and attitude etc. Anybody who is not completely focused on the patient in front of them needs to be reminded that a simple courteous attitude of service forms the basis of every encounter.

It is of course extremely important to hold your ground on policies and procedures but to convey them with genuine passion. Again, by making the benefits obvious to your patients, your entire practice benefits.

Needless to say, this is an extremely powerful time a year to reinforce these concepts with your entire team. We suggest you weave this through all holiday events you may have planned for the rest of the year.

Also, this is the perfect time to examine your own management systems, to measure their own user-friendly characteristics. Ask staff for input in terms of fine-tuning communications with the entire team for better execution.

Really take some time with this. Incorporate some of these simple ideas into policies and procedures, and regularly check to make sure they are being completely implemented. You will reap the benefits many times over.

What Can You Control?

As the tendency towards overwhelm occurs in so many areas of modern life these days, building a compelling future, no matter what the past, has certain fundamental requirements of you as an executive and architect of your own future. You and I cannot control what’s going on in the outside environment, but we can control what’s going on between our own two ears.

All of us can take a very proactive stance with our own head space. We can educate our patients. Better train our staff. Fully educate our communities.

But in order to do really do this, you cannot come from fear. You cannot come from lack. You must make sure that you are coming from your highest purpose when you enter the office every single day.

One of your highest purposes has got to be your own financial freedom and your own unique prosperity.

It’s important that you take steps now to make sure that you’ve solidified the financial income to your office and the amount of personal savings from your practice.

I recently spoke to two docs back to back from similar parts of the country. They were in the same age group. One doctor had been in practice about 15 years and the other about 12 years. One doctor was doing extraordinary well. The other was absolutely going down the tubes.

What could possibly account for the major differences?

I investigated a bit further. What was happening was that the doctor that was having a difficult time had failed to respond to warning signals many months earlier. Certain third parties came in and basically said, “There is no more coverage for X, Y and Z.” His entire practice, which at one point in time was an enormous practice, was suddenly shut down because he had nothing to fall back on. He had no other pathways.

To bulletproof your practice and future in any economy, you have to maintain a very close and diligent watch on your vision. Along with this, you must also make sure that you have developed the personal time to reflect and investigate, and the necessary administrative time in your office to deal with all these things.

It does take time to implement and perfect real world procedures and train your staff, but unless you actually build this development time into your practice, you’re going to have a difficult time with what’s coming down in the future, regardless of whether or not you participate in insurance.

If you have been at this a while, you, as well as I, need to continually remind ourselves when we hit some of these frustrations that practice is radically different now.

I continuously remind my team to come from a position of acceptance, calmness, recognition and figuring out new and better ways, and to how to help private practice prosper. This type of approach makes an enormous difference.

The doctors that have taught themselves to engage in this behavior have restructured their personal lives and office hours, so they have the needed admin time. They have found it enormously beneficial.

There are just some things you need to be fully cognizant and aware of what it’s going to take to continue to bulletproof your practice, and indeed your private life.

To that end, remember it’s a work in progress.

Private Practice Owners and Online Reputation Management

It Just Takes Guts

Guts and determination. Powerful words. But why should they be used in the same vein as healthcare? Because of the world we now live and practice in it just takes guts to say what you believe is best for a patient, on so many levels.

It takes guts to say what you believe is best for a patient, on so many levels.

It takes guts to buck the status quo with our patients and their families, to tell them the newest designer drug is not going to cure their obesity or poor health habits.

Contrary to the incessant drug company advertising, life will never be all be flowers, endless orgasms, smiles and happy fields of dreams due to the newest ED med or antidepressant, if patients don’t make some critical life choices for themselves.

It takes guts to tell it like it is, to really separate the valid life advice from the endless barrage of crap hurled at patients day after day.

This however, I believe can be the only true way out for all doctors in private practice today. And a big win.

For everybody.

Society will win. Not all of it, of course but much bigger segments.

Crisis based health care expenditures would drop like a stone if we helped to prevent, or at the least reduce the complications from heart and vascular disease, depression and chronic back pain, diabetes to name only a few were to decrease even just a little.

And doctor, if you are in alignment with my philosophy, continuously developing your perfect practice, you are already well on the way to making this a reality.

And what happens if we all practice this way?

Once again physicians of all disciplines become the real decision makers. We decide collectively to continue to be teachers, and leaders by example. Refuse to cave to third party based interests and administer only to the patient’s needs first. Not the insurance companies, drug companies or hospitals.

To truly stay Commanders of Change™ will takes guts.

Simply, just being good doctors first, one patient at a time, regardless of which initials you have at the end of your name.

But, to do this for any period of time requires very powerful support systems behind you. It can be stressful, unfulfilling and eat you up otherwise. As a consultant, I see this firsthand.

First of all, your beliefs (guts and determination) and personal lifestyle. Beliefs about what constitutes true health, self-reliance, responsibility and dependency.

And money. What is your care and advice really worth? Do your fees reflect this?

Secondly, a staff who supports your vision.

Lastly, powerful and extraordinarily effective systems and technology.

But doctor, none of this can ever work without a powerful philosophy that you can communicate.

Patients must know what your about. They need to feel it. I’m not saying you should be flamboyant or radical.

In fact, this turns way too many away from you whom might otherwise benefit from your care. But I am saying you must clearly stand up for what you believe.

Never forcing, but presenting, supporting and helping those patients and staff who can align with your purpose and vision.

And gently getting the rest out of your life.

It just takes guts.

But, here is the reward. In the long run, you will be much happier, more prosperous and healthier yourself. You will help tons more people. Have a much broader reach.

And patients, and indeed entire communities who are wanting to personally pay to see only you.

Yes, even in today’s world.

Transitions

Transitions…

I think one of the most powerful but sometimes scariest things we do as physicians caring for patients, but also mentors to students and younger docs is help them create a compelling future life, and then actually watch it start to unfold right before our eyes.

Think about it. As a doc, you did this too, first as a student at several levels. In reality, many times. In fact, at one point it was probably term-to-term.

Then, you began to practice. Still way more changes. Lots of excitement, new ways of doing things, patients that challenged every skill you thought you forgot.

So, in reality, you know you have the power to create magnificent changes.

Getting Stuck…

After a while, what was once magnificent and exciting can seem dull, boring, and very routine. We sometimes so easily forget the sacrifices we made to get where we are. High school, under-grad, and on and on. If we allow it, the daily grind can become just that. If it’s not practice stress, it’s kids, family, etc.

Then sometimes we build fences, stop communicating with past friends, we can also become very rigid in certain habits, most especially mental habits and sometimes getting really, really stuck.

Before we know it, seems like years have passed.

If things did not turn out like you thought, it’s easy to just give in, and say, “…Oh well…” We in fact, can start to live by default.

That’s when its not pretty…

So what is the most simplistic solution? Respectfully, our role must be to maintain our own health and happiness first. This is the only way we will be able to carry forward our roles as physicians, on our own terms. As long as we choose.

 

Decisions & Plans Create Reality

The Decision To Move Forward

The decision to move forward. This, unfortunately, still remains a major stumbling block of many. Indeed, the slowness or lack of making even simple decisions, but most especially putting off critical decisions sometimes slows forward progress by weeks, months, and in some very unfortunate circumstances years.

But I believe that the reason this most frequently occurs is a lack of clarity, a lack of vision. All too often, in general people, even ourselves as highly intelligent doctors, are unaware of the vast opportunities that wait in our personal and professional lives, simply for the asking.

Those who are happiest, most prosperous and effective realize that it is this willingness to take action in meaningful directions on moment-to-moment basis is the ultimate in accomplishment.

Doctors, the decisions you make today will have profound impact on next year. And the direction that you take professionally as well as personally next year will also have very significant impact not only on the rest of your professional career, but as well on the quality of your personal life.

Right now, is simply the very best time to reflect very carefully on what this past year worked right, what needs to go, what needs to stay.

The reason for these rapid and effective decision-making skills set is frankly is that the speed of change is ever accelerating.

There has never been a better time to be in private practice, but only for those who possess clarity, a phenomenally well written game plan, remain strong, effective communicators, capable and willing to provide only the very best in health care services.

But there’s something that’s extraordinarily important to clarify first, and that is your personal vision.

First, (not last) the quality of life you want for yourself, as well as those closest to you.

Reality dictates, that this future style of private practice will require breaking with the rank and file. Future practices will require blending all the best technology has to offer, the very best in cost effective systems and procedures all combined with time tested humanistic skills, and compassion.

It will simply not be enough to have pieces of this puzzle. Rather, all pieces must fit together with exact precision.

Therefore, I respectfully suggest that you start assembling your puzzle now.

And in doing so, carefully consider all ramifications of your decisions. This is where exercise, quiet time, reflection as well as inspirational reading and study while developing fully your game plan, all carefully written out and diagramed on paper will make a tremendous difference not only in how you feel, but how you act.

Once you have clarified this new vision, it is essential that all the steps you wrote as necessary to actually pull off its execution are first set into motion, and put into written timetables.

Then, perhaps most importantly choose your support system.

In my experience, it is those doctors who fully utilize great support systems, accomplish far more throughout the year than those who do not.

The best support systems provides you with all the information you will need, the personal as well as professional associations, camaraderie and affiliations that supply the building blocks, systems, plans and tools necessary to complete your puzzle.

But only YOU can reach out and grab it.

 

Changing Spaces: Time to Really look outside your practice?

Having just spent the better part of the week working with aggressive entrepreneurs in private healthcare I can tell you that everything we have been saying since 2005 about technology integration into daily practice  is being accelerated at an even more rapid pace.

Here’s a fact. Patients ages 55-90 are now the biggest social media and internet “geeks”.

They love someone, they post it. But piss them off, and look out. And you need to exactly how to protect yourself and what to do if this happens to you.

Creating A Compelling Future

Why not you?

Recently, especially as the private healthcare environment harshens, I have been asking myself much more often about the real differences between those who are wildly successful, and those who struggle, year after year in practice. Why not you?

This really hit me hard after I re-listened to our own most recent webinar on Perfect Practice for prospective members. Frankly, I sometimes wonder what makes those who take our programs, implement and follow them and become wildly successful different than those who don’t.

Frankly, It hit me again just this morning after a fabulous weekend, why anyone would even expect to be successful, or better yet become truly independent, without careful attention to every aspect and area of business, success and personal achievement. The answers came to me with blinding simplicity. So, I made a list of the top three reasons for failure to thrive in practice. And you know what? Its just three things!

Those who become very successful in life and practice do three unique things that those who struggle don’t.

1. Successful people totally and unconditionally accept full responsibility for their lives and practices. They never blame the economy, insurance companies or patients who just don’t understand.

2. The most successful are continuously gathering new information. These are the readers and seminar takers. Theses doctors take every call we sponsor, request more help, frequently with letters and emails. They travel to every live event we give or advise them to take. They regularly gather all the knowledge they can. These doctors completely understand that knowledge is power, and the ultimate in personal freedom. They have a book or audio on success handy, at all times. It makes no difference if they are in year 3 or 30 in practice.

3. Lastly, these doctors know what they want. They can spout off their goals and aspirations with great clarity. And when their mental pictures become cloudy, they pause, reflect and redirect. Ultimately, these doctors make their own decisions, after gathering all the data, and truly decide what’s in their own best interest, as they tweak and fine-tune their game plans. To the core, they are internally focused, but ever vigilant to support in every way possible those who mean the most to them.

Doctors, the good news is changing from mediocrity to complete fulfillment and happiness is as simple as hitting just these three things.

But, what if you are overwhelmed with challenges? What are we to do first? What do those who are ultimately hugely successful in life do?

Ask successful people for guidance. They never fail to share and help.Always ask for help and guidance from those who have been there.

Successful people engage in all the critical aspects of living a self-actualized life. And they tell you just how they get it all done. This is the reason that continued immersion in success skills and philosophy; personal growth, physical fitness and spirituality all are essential components of Practice by Design™.

Remember as great as spouses and families kind support can be while you are stressed, nobody really knows what you face in practice except peers. So, when it’s really critical information you need, do you take advice from the most successful, or not? The answer should be obvious.

Next, practice total and unconditional acceptance of personal responsibility.

If you have not done this, you may need to work on this. Daily. This where reading the masters, and spiritual works such as the Bible, the Gita, Koran or simply going to that quiet place within your own soul really helps.

Lastly, if things aren’t working for you in any area of life, make a new plan!
Today!

Remember, the definition of insanity is doing the same old thing in the same old way expecting different results. And as the world changes, our plans often need to adapt or change as well. Realize, this is exactly why I repeat our Core Modules for members live, every six months. Because the pace of change is now so rapid.

And, remember Future Shock? Remember how easy it is to be mesmerized in the face of rapid changes, and the tendency to shut down and do nothing, rather than taking on a new game plan.

Now, here’s the really good news. These are behaviors, and learned choices!

So, here’s a simple plan you can act upon right now to be happier, more successful and fulfilled,

1. Be a better and bigger asker! Jim Rohn says the problem with most of us is that too many metaphorically go to the ocean to gather water with a thimble, instead of a bucket. Be a better and bigger asker and life rewards in kind!!!

2. Continually engage and learn! Not just practice, but all of life’s most important skills. Be willing to pay the price of success and then reap all its joy.

3.Make sure you know what you want, and continually move towards it!

Lastly, burn all your bridges to past failures. You will be glad you did.

I have been asking myself much more…

Avoiding KEY Reasons for Failure

I have a short list of reasons for failure, when a prospective client comes to me. I have to tell you that the vast majority that ultimately become clients are successful because I personally vet all my clients on some really key issues.

There are reasons for failure. Number one on my list is something I hear all the time.

I tell Kerri, “If you ever hear this from a doctor, you have to tell them that we just can’t help them.”

That is when a prospective doc  says  “All I want is the marketing”.

I can’t tell you how many times I hear from prospective doctors, “Nope, I have everything else down. All I want to know about is marketing.”

I will tell you right now that when I hear that, virtually without exception, I can predict that the doctor will go down, usually in a ball of flames within a relatively short period of time.

It’s not just about the marketing.

It IS KNOWING how to IMPLEMENT SYSTEMS. And Keep them all running just like we teach you how.

Getting every step of private practice to run like a Formula One Racer…

The fact of the matter is frequent, relevant communications and contact with patients, the community and your referral professionals is paramount to your success, and will be even more so in the next 9 months!

Will YOU be part of it???

It’s as I said when I did my first live event in Boston many years back. If your patient takes out their handheld or and you don’t come up on the first page, you don’t exist in the minds of at least two generations of patients. Too many doctors still fail to come to grips with this!

So, with all that said, here’s how two of our clients talked to us about HOW they are getting it all done…

From Dr. Mark Giust, of Brookhaven, Mississippi

“My biggest challenge was just trying to take some action steps. What you helped me with the most was just realizing that you don’t have to take humongous steps. You can take some small steps to help you move in that direction and not just sit still. Everyone knows sometimes what they need to do. It’s just a matter of getting out there and doing it and doing it the right way that can help them be successful.

I think sometimes we have a lot of choices to make. There’s a lot of information out there to sort through. Before working with you, I was overwhelmed with the amount of information out there. There’s webinars and CDs. I think sometimes I was afraid I was going to miss something. I’d try to listen to all of it, but then I’d almost get overwhelmed and think, “What do I do?” I’d jump from one thing to the next and maybe not stay on the right thing.

We talked about practicing by design. You need to have that base to start with and build upon. You need to do the most important things first. You’ve helped me a lot with staff issues, part-time help and utilizing my time more efficiently.

The biggest point you made that I want to make sure to emphasize is that the very first thing you did was take our Core Module 1 and literally redesign how you wanted your practice. We spent the rest of the year working toward that goal.”

 

Next, Dr. Robert Cole & Jennifer, Monroeville, Pennsylvania talked about Priorities…

 

“It is better when you’re constantly implementing and taking action steps, but they’re smaller and more comfortable. You gain confidence by doing things like newsletters and health talks.

Action steps is one, and confidence with patients is another. It was one of your newsletters, or it might have been an audio CD where you talked about how sometimes you have to fire a patient. You may have to say, ‘You’re not doing what I asked you to do. It’s a health risk for you. It’s a liability for me. We need to terminate this.” A lot of times, it’s a relief. It’s like cutting out the poison from your practice. If they do stay, it invigorates your relationship with them, and sometimes the whole practice overall.

I don’t know if most of you are aware of the movie out called “The Blind Side.” The author goes back and forth explaining things about football. They put various quarterbacks in when Joe Montana and Steve Young got hurt. That system was so effective that they could put almost any quarterback in there and turn him into a star because the system worked.

It instantly reminded me of what you teach and talk about. Once you have the systems in place, you can walk into that scenario and have it work effectively in an ethical and moral way.

I think organization and self-confidence were really big issues. What we’ve been doing with you walking us through the steps has helped us to regain our confidence. It was always there, but we had to build it back up. We definitely did that.

It’s comforting to know that we have somebody in our court. We have a cheering sections. One of the big things is consistency, whether it’s working out or eating right or working on your practice. It’s doing the right things consistently. It’s planning out ahead of time for short and long-term stuff. It’s sticking with a plan and continuing to keep your head in the game

“We’re all in the same boat. You offer some real-world solutions. It’s not that much of a struggle to implement them. I’ve worked with some people and it was such a shift in what we normally did that it’s a real stressor. This isn’t a stressor. This is getting your ducks in a row.”

What Do You Stand For?

It has been said, that “If you don’t stand for something, you will fall for anything”.

To be truly independent, on this great day of American Independence you need to know where you stand on the major cornerstones of your life (Personal, Spiritual, Material, Fitness and Financial).

A strong personal philosophy on each of these is vital to your success, and developing an evolving professional life and practice .

This is a great day to ask yourself if are truly living the personal and professional life you deserve.

Are your actions leading you in the direction of the goals you set at the beginning of the year?

If not, make the midyear corrections NOW to insure not another precious moment of your life is wasted! (This is one place where one on one consulting can really help this to take shape for you!)

When I think and read about what our American forefathers accomplished, (while spending the Fourth on Nantucket, steeped in American History), especially in light of the challenges they faced, most of the tasks we have at hand seem to pale in comparison. (I firmly believe this is also why America needs to continue to be clear on what we stand for!)

What do you stand for? Make sure your patients and your staff know your personal philosophy on these vital cornerstones or you are liable to ‘fall for anything’!

If you are not crystal clear on your personal philosophy, or even if it needs some updating or fine tuning, get out a blank legal pad, sit in a quit place, close your eyes for a while and list what comes to mind. Just make sure you do it before you go to sleep tonight!

All successful people can clearly define their stand.

You must count yourself in these ranks.

Private practice mandates an entirely additional set of skills today. More so than any other time. Not instead of but in addition to your healers skills and instincts.

And honestly, failure to realize this, or especially educate our students creates every unpleasant and often painful experiences ranging from unrest to bankruptcy.

Interestingly, with patients we tend to be extraordinary analytical, follow algorithmic, logical sequences, and frequently just one or two steps at a time.

It’s this thought processes that lends itself to accurate diagnoses and rapid, critical action.

But private practice demands an entirely different set of skills, especially when marketing, finances, and staffing are concerned. Let alone technology, human resources, your web-facing efforts, and wealth building.

In fact, modern private practice demands an entrepreneurial mindset. The days of just being a great doc without , marketing and management and exceptional business skills is, with rare exception, over.

This is why despite everything else you may do, you have got to have an excellent business training to fall back on again and again.

Times are never good or bad, only different.
Sage advice from a master, spoken many years ago.
I am not sure exactly who or I would give due credit.

And you know what? It’s very Darwinian, especially today.

Only the fit will make it. And
come out so much better. I know this as I speak
with doctors all over the USA.

This is a great time to reinvent yourself
personally, spiritually, and refine your practice
make-up, vision, goals, and especially your
systems.

So much is happening so fast right now.

Just ask your patients.
Ask them what they need. What they want from
you to achieve their own objectives.

And by asking this, you find out how good
a job you have done educating your patients.
Or not.

Let’s take a look at these skills so essential to phenomenal success but so often overlooked.

Number one on my list is the ability to organize and sort incoming “stuff” and data.

Really.

A messy desk (or home or personal life quite frankly) makes for a clouded mind. That’s why extreme and easy organization is the first lesson in Living & Practicing by Design

Along side of this skill, you also need to be able to organize, focus and direct output and productivity. This includes everything from processing new patients to getting the income to run, gwo and market your  #privatepractice, and especially build long-term security.

Think about this one. There is not one area in practice where the rule of this skill set does not apply.

The second skill set is just as critical.

And that is developing a CEO/CFO mindset that you’ll have to use right along side the Dr. before your name.

This is perhaps the one that takes the longest to develop for docs. But like everything else, just a little regular study leads to mastery.

Are your patients getting mixed messages?

Are your patients getting mixed messages, and you don’t even know it?

Most especially, might there be an incongruence or two with regard to office policies which might not only be slowing you down, but could be like driving your car with the brakes on?

Well, if you have more than one staff person, the answer might be yes. It’s so important to try and head this off with the policies and procedures covering every little detail.

Even in our practice, I discovered this quite by accident last week. The problem began with a newer staff person getting into encounters for the first time by herself.

It seems like everything has broken in our office in the last 3 months. Computers, phones, credit card swipe machines, and even a diagnostic unit. Stay in practice long enough and stuff just happens to fail eventually.

Anyway, last week it was the card swipe machine. This can really hurt the income for a day or two as you know.

Now my staff is very bright, and they know our policies on collections. So why did the failure of one simple device cause chaos at the front desk?

It was simply because the basic “fix” that is in the office manual was not referred too. Once I pointed this out, we were back to normal quickly.

Make A New Plan

If things aren’t working for you in any area of life, make a new plan!

Remember, the definition of insanity is doing the same old thing in the same old way expecting different results. And as the world changes, our plans often need to adapt or change as well. Realize, this is exactly why I repeat our Core Modules for members, every six months. Because the pace of change is now so rapid.

And, remember Future Shock? Remember how easy it is to be mesmerized in the face of rapid changes, and the tendency to shut down and do nothing, rather than taking on a new game plan.

1. Be a better and bigger asker! Jim Rohn said the problem with most of us is that too many metaphorically go to the ocean to gather water with a thimble, instead of a bucket. Be a better and bigger asker and life rewards in kind!!!

2. Continually engage and learn! Not just practice, but all of life’s most important skills. Be willing to pay the price of success and then reap all its joy.

3. Make sure you know what you want, and continually move towards it!

Lastly, burn all your bridges to past failures. You will be glad you did.

Doctor, Are We Focused on the Right Things?

Doctor, are we focused on the right things? Honest to God, its 3:10 AM. I can’t sleep, but I feel so lucky. One of my patients died last week. My wife of 30 plus years is asleep next to me. How lucky am I?

Judy presented to me about 10 weeks ago with acute sciatica, and low back pain.

At first, it seemed simple enough. By the end of week one, I sensed something completely different. Nothing gave her anything but very passive relief, nighttime was unbearable. Then she developed significant weakness of the legs and gait changes. Interestingly, her neuro signs were still benign, but that changed oh so quickly within 48 hrs. Something was “bad wrong”, here as one of my earliest professors would say.

He husband who I have cared for for 25 years was with her every moment. Very concerned. Scared for Judy.

I called her PCP who immediately began a hospital intake after we both voiced concerns about unrelenting night pain and progressive leg weakness now justifying admission and a complete workup.

Judy went down-hill fast. Initially, what started as a simple sciatica was next thought by her neurologist to be Gullien-Barre’ Syndrome.

When medication (steroids, etc) did not help significantly she then quickly had plasmapheresis, and was sent to transitional care. She never improved significantly.

Very quickly, it was established she had some very aggressive non-specific demyelinating disease.

In essence, all this happened inside of 3 months.

Judy and Bob were married for 50 or so years. Over in a flash. There is nothing any of us as her team would have done any different, or better. But, it still sucks.

Of course we see this as part of practice, and way more if you spend a lot of time in hospitals. I did four years of this type of transitional care, and I still see the face of the pretty 23-year-old girl dying of a brain tumor, and many others 30+ years later.

So, yesterday morning, after my staff broke the news (we had called Bob the night before to check on Judy), instead of bitching about how so many obstacles are being hurled onto the path of us in practice, I felt really lucky. I immediately thought about my wife and daughter.

For some reason next I start thinking this morning about the doctor from an insurance carrier trying to lecture me on another patient last Monday morning on absolutely ridiculous daily record keeping, way beyond any standard of care. I told him so. I also told him I would gladly perform a high level encounter at each visit as long as it was paid for. I also explained that the company he reviews for denies such services as “not medically necessary”, so this will cost the patient an additional 150 or so out of pocket, for each flare up of the SAME condition.

Huh?

And I am thinking this is just nuts. There’s no common or rational clinical sense in this at all. This is not good chiropractic or good medicine.

And I told him, “Screw all of this. Patient care always has to be the most important thing here”.

He’ll never get it.

A year ago, it still made sense to work for these people. I’m still #lovinghealthcare

But, if this keeps up they are history in my book.

Because when its done, that’s all there is.

We speak often about the power of planning, and then implementation.

All fine, and essential. But happens when your plan does not work as intended?

There is a saying in marketing that there are no failures, only tests. It’s all just a “test”.

And so it is in the world we live in right now. What worked so great in practice a while back may not be working so well right now.

What do most people do? Stop trying!

What’s a better way?

Analyze, fine tune, make some appropriate changes, and “test” again.

Then again.

Seriously. Every great practice and business is built upon the backs of many, many failed “tests”…

“Now what do I do?”

Doctor, if you have asked yourself  ‘Now what do I do?’ even once in the last week, something is incorrect, broken or missing in your game plan.

Your practice is strong, your investments are worth more not less and you lead your team with eager anticipation of the future by example.

If not though, a really critical appraisal of EVERY system in your practice, and probably personal life is long overdue. You should be doing this every six months at a minimum anyways.

Here are some key issues to consider right now.

Have you made your patient care plans simple, understandable by your patients? Or, are they confused, and when it comes time to pay say all too often “Let me think about it.” “I can’t possibly afford THAT right now.” And you watch them drive off in a better car than yours.

Are you asking your patients what they want and going out of your way to provide it?

And your team. Are you reassuring, or frightened? Are you complaining about how much money you either don’t have or lost, or are you supportive, and doing everything you can to ensure them a bright future? Do they absolutely know where they stand on everything about your practice??? Or, are they spooked because you are taking calls from your broker when with patients.

Are you making them feel confident, or scared??? Do they talk about you, and in what vein? Are you working every week to make their jobs easier or more difficult and confusing?

Let’s talk money.

Are you spending money on your continued growth and education, or did you axe that too. Will you be joining us to learn from some of the brightest people in healthcare, EMR, marketing, interdisciplinary care and law right now? Are you fully engaged in continuous self-improvement? Books, CDS, podcasts, teleseminars and webinars?

And what about marketing? Still losing money on print ads, or does YOUR name come up when they Google their health concerns. Is it YOU on the local radio health shows, or your competitors? Is your website up to snuff? Are there ways on YOUR website to capture New Patients on-line? The Same Day? Are you targeting your marketing, or broadcasting dollars into the wind?

While writing “Living and Practicing by Design”, little did we know at the time how quickly much of what we wrote about would be a bullet proof strategy against the ever changing economic and practice landscape.

The good news is, that’s what we do together in our memberships.

And doctor, based upon the events of the last few weeks, you will need to create all of this stuff in your own life.
